Reporting to the Manager of Outreach Services, and applying a harm reduction philosophy, the Outreach Nurse is responsible for delivering integrated, comprehensive community-based nursing, advocacy, support and referral services, to identified individuals and families of the Kwanlin Dün Health Centre catchment population and others who are homeless, at risk of becoming homeless, living with addiction, mental illness, poverty and/ or trauma as well as clients involved in the sex trade. The position works collegially with the Kwanlin Dün First Nation (KDFN) Health Centre staff and other KDFN departments along with a wide variety of local, territorial and federal organizations ensuring that the best culturally appropriate care is being provided to vulnerable populations who are accessing healthcare.
Required Qualifications (what we screen/look for on your resume): An equivalent combination of post-secondary education (i.e.: Diploma), combined with significant current/lived experience in the areas identified above, will equally be considered.
- Bachelor's degree in Nursing from an accredited institution, including current registration in good standing with the Yukon Registered Nurses Association.
- Significant nursing experience, preferably in a First Nation community/setting; experience within a trauma, violence, abuse, harm reduction, mental health and substance use disorder setting is recommended;
- In-depth knowledge of and experience implementing harm reduction practices and Hep C/ HIV prevention
- Demonstrated competence in clinical nursing practices related to the care of adult vulnerable persons;
Knowledge and skills (what we may assess/interview on):
- Assessment, planning, intervention and evaluation skills to support nursing practice and client care planning.
- Knowledge in immunization administration and STBBI sample collection.
- Ability to perform appropriate wound care assessments and treatments.
- Case management skills and abilities, with the ability to effectively manage a considerable caseload.
- Ability to demonstrate conflict resolution skills and assist in crisis situations in a calm manner.
- Demonstrated teaching and mentorship skills to effectively support others. Promote team collaboration.
- Ability to develop therapeutic relationships and effectively communicate with a diverse, integrated. multidisciplinary team and the public.
- Knowledge of KDFN history, culture, demographics, families, social dynamics prevailing in the community, our goals and aspirations.
- Current Yukon Registered Nursing Association license, and requirement to be in good professional standing with YRNA
- CPR & First Aid; Transportation of Dangerous Goods (TDG) Certificate
- Food Safe Level 1 (Willingness to obtain with 3 months of hire)
- Satisfactory pre-employment Criminal Record Check with vulnerable sector search
- Valid Yukon Class 5 Driver’s License – Class 4 is desirable, and current driver`s abstract
- Immunization Record &, TB Test upon hire
- Certificate of Immunization Competency (maintenance of this certificate is a requirement) Yukon Vaccine Certificate (or willingness to obtain within 3 months of beginning employment)
Knowledge of Yukon First Nations history within formalized education systems, residential schools and on-going intergenerational effects specific to education and social programming is an asset. Familiarity with Kwanlin Dün First Nation history, culture, goals and aspirations is required for success in the role.

Salary: $76,531.00 - $120,811.60 per annum ($42.05 - $66.38 per hour) Level 7 *plus benefits
