Salary:  Level 7 Step 1 ($3,316.50 bi-weekly)

Reporting to the Health Programs and Services Manager, the Home and Community Care Coordinator is responsible for the implementation, coordination, and administration of an effective and efficient home care support program for citizens / clients in need of assistance. This position ensures citizens / clients have the best possible quality of life while facing medical, physical, or mental health challenges limiting their ability to perform necessary home and self-care functions. The incumbent meets these objectives through effective program and service delivery, using a collaborative case management approach to providing services and supports, and by ensuring that TH culture and traditions are included in program design and delivery. 

An Eligibility List may be established from this competition – the duration of the eligibility list may be up to 12 months. The eligibility list may be used to fill future permanent full-time, permanent part-time, temporary full-time, temporary part-time, term, or auxiliary on-call vacancies within the same department and classification based on the organizational needs by going the next highest ranked candidate until the eligibility list expires.

Essential Qualifications:  

  • Post-Secondary Diploma / Certificate in Home Support Care, Home Care Services, Home Health Aid, Long Term Care Aid, Nurse Assistant or a related field of study. An equivalent combination of education, training, skills and experience may be considered.
  • Demonstrated work experience in community home care or health support care and in implementing case management planning and practices.
  • Demonstrated experience in program / event / workshop administration including planning, facilitation, presentation, and budget management.
  • Experience in scheduling, training and supervising staff members.
  • Knowledge in NIHB processes and procedures and ability to assist with medical claims / reimbursements.
  • Knowledge of, or willing to learn, Tr’ondëk Hwëch’in Government structure, culture, traditions, history, demographics, family structures, socio-economic dynamics, challenges and aspirations, basic phrases of the Hän language, as well as the intergenerational impacts of residential schools on individuals, families and communities.
  • Ability to advise and guide community / staff on home care services.
  • Exceptional interpersonal skills, ability to develop and maintain positive relationships with the community, citizens / families, co-workers and other departments and outside agencies.
